Wall & Ceiling Threats Guide
How to handle Wall Bangers, the Don't Look Up ceiling threat, Wall Anomalies, Surgery tentacles and green sludge in Animal Hospital.

Not every threat is a Skinwalker or a creature under the bed. A whole family of hazards appears on the walls, ceilings and floors of the hospital, each with its own specific counter. This guide covers all of them in one place. Remember the golden rule: weapons only work on Skinwalkers, so every threat here needs a non-combat response.
Wall Bangers
Wall Bangers appear randomly and bang their heads on your check-in room glass wall. This is more than annoying — it prevents you from rejecting Anomalies by blocking the Shutter, which can let a disguised patient through.
- Correct counter: approach the Wall Banger with coffee or other food and interact to remove it cleanly.
- Costly alternative: if you interact without coffee and hold E to make it leave, it works — but you take a lot of Sanity damage.
So keep coffee or food on hand specifically for Wall Bangers, since they directly interfere with your most important job.
Don’t Look Up
The Don’t Look Up threat spawns randomly on the ceilings of treatment rooms. Its counter is in the name:
- Keep your camera down and finish your task without looking up. It eventually disappears on its own.
- Looking up at it deals Sanity damage.
When you enter a treatment room and sense something overhead, resist the urge to check — just keep your view level, complete the treatment, and let it vanish.
Wall Anomalies
Wall Anomalies spawn on walls and chase you if you are close, approaching to hit you for Sanity damage or to steal items.
- Correct counter: aggro it and then run away. Wall Anomalies disappear after a while, so you do not have to fight them — you just have to outlast them by creating distance.
Do not waste time trying to attack a Wall Anomaly; lead it away from where you need to be and let it despawn.
Surgery Tentacles
Surgery tentacles spawn in room 8 during Surgery. They are the most counter-intuitive threat in the game:
- Do not shoot or taze them — attacking the tentacles makes your patient die automatically.
- Just finish the Surgery mini-game while avoiding them. Complete the operation and they cease to matter.
This is the opposite of how you handle a Skinwalker, so commit it to memory: tentacles are out-lasted, never fought.
Green Sludge / Slime
Green sludge (or slime) is a terrain hazard on the floor. The counter is simple:
- Hold E to clear it.
It is the least threatening hazard on this list, but clearing it keeps your paths clean as you move between the desk and the rooms.
The common thread
Every threat here proves the same point: in Animal Hospital, the right response is specific and usually not combat. Coffee for Wall Bangers, eyes-down for Don’t Look Up, run-and-wait for Wall Anomalies, finish-the-job for Surgery tentacles, and hold-E for green sludge. Drill these until they are automatic and the hospital’s environmental threats stop costing you Sanity.
