Stalker Guide

How to deal with the Stalker in Animal Hospital: it spawns around corners and damages your Sanity if you look directly at it. Never make eye contact.

Last updated: June 19, 2026

The Stalker is one of the hospital’s pure avoidance threats. It does not chase you down or need an item to remove — it punishes a single mistake: looking directly at it. Knowing it exists and where it appears is most of the battle.

Where it appears

The Stalker spawns around corners. As you move through the hospital between the reception desk and the treatment rooms, it can be waiting just around a bend. The key is to watch for it from a distance so you spot it before you are right on top of it. Awareness is your early-warning system.

How to deal with it

The Stalker has one rule:

  • Never look directly at it. If you look right at the Stalker as you pass, it damages your Sanity.

So when you spot a Stalker around a corner, keep it in your peripheral vision and move past without locking your camera onto it. Do not stop to stare, and do not turn to face it. Treat it like something you have noticed but are deliberately not acknowledging — eyes forward, keep moving.

Why weapons don’t work

As with all environmental enemies, weapons are useless on the Stalker — they only affect Skinwalkers. There is no way to “kill” the Stalker; you simply deny it the eye contact it needs to hurt you. This is why it is categorized as an avoidance threat rather than a combat one.

Building the habit

The Stalker rewards calm, deliberate movement. New players panic when they round a corner and see it, instinctively staring straight at it — which is exactly the wrong reaction and exactly what costs Sanity. Train yourself to do the opposite: when something appears around a corner, glance to identify it, then keep your camera angled away as you pass. Combined with watching from a distance, this makes the Stalker a non-issue.

Part of a bigger pattern

The Stalker belongs to a family of “do not look” threats in Animal Hospital. The Don’t Look Up ceiling threat works on the same principle — keep your camera down and it cannot hurt you — and the camera dark-figure zoom similarly demands that you look away. Recognizing this pattern helps: when the game presents something unsettling, your default reaction should often be to not look at it directly while you continue your task.
