Beating Animal Hospital is not about a final boss — it is about surviving deeper and deeper shifts while keeping a clean scoresheet. The players who go the furthest are not the fastest; they are the most consistent. This guide covers the late-game tactics that separate a shift-3 wipe from a high-score run.

The high-score formula

Everything that earns score also keeps you alive, so playing for score and playing to survive are the same thing:

  • Reject every Anomaly. Run the three-layer routine on every single patient, no exceptions. Missed Anomalies become Skinwalkers that kill patients and drain Sanity — a double scoring hit.
  • Cure every genuine patient. Take your time in the treatment rooms and match items correctly. A single wrong item can erase a clean shift.
  • Lose nothing. No dead patients, no Sanity wipe. Survival to the payout is what compounds your cash.

Detection is a memory game

The reason consistency matters is that the game reuses categories of tells even as the specific patients change. As you climb, keep a running mental list of every Anomaly you have caught and what gave it away — three eyes, a black-box camera feed, a static photo. Players who treat each new Anomaly as a lesson improve fast; players who rely on luck plateau. By the late shifts, your detection should be near-automatic so your attention is free for the chaos.

Dealing with Skinwalkers that slip through

Even great players let one through during a hectic event. When that happens, switch from detective to defender:

  • Weapons: approach with a gun or taser and use M1 attacks. This is the safest, fastest option and the reason you keep a weapon in your kit by the late shifts.
  • Bare hands: if it is attacking a patient, hold E from behind it. If it is on you, hit E repeatedly — but you will take Sanity damage, so this is a last resort.
  • The wrong-item kill: if the Anomaly is in a treatment room, giving it the wrong item kills it with no point penalty. This is the cleanest cleanup of all.

Surviving the Ambulance rush

After shift 4, the Ambulance event can trigger, flooding the hospital with patients all at once. Some arrive as patients on fire (timed emergencies), some are normal, and some are Anomalies that auto-admit and will awaken into Skinwalkers. The key is preparation: by shift 4 you should already own a weapon and have coffee in reserve. When the rush hits, triage the timed emergencies first, then hunt down the auto-admitted Anomalies before they hurt anyone.

The Barney storyline

After shift 2, the Barney event can begin — a multi-stage encounter with a white-rabbit character who is secretly a serial killer. You can reject him outright or play through his choices for a reward: hide him from the police and he gives you a second coffee machine, a huge boost to Sanity management. Barney can also turn into an Anomaly, which you take down like any other. Decide your approach before he arrives so you are not caught off guard.

Late-shift mindset

The deeper shifts layer more anomaly types and more simultaneous events on top of the basic loop. Do not let the volume rush you. Keep the same calm routine: check in deliberately, treat carefully, respond to emergencies immediately, and keep Sanity topped up. Pre-buy your weapons and food so you are never scrambling mid-shift. Master that, and there is no shift you cannot reach.
