Barney is the standout storyline event in Animal Hospital — a multi-stage encounter with a white-rabbit character sporting black slit eyes, who turns out to be a serial killer. Unlike the random emergencies, Barney is a choice-driven event with a real reward on the line, so it pays to know how it plays out before he arrives.

When Barney appears

The Barney event can trigger after shift 2. When it does, Barney comes into the hospital like a patient, but his storyline unfolds across multiple stages rather than a single check-in. You can engage with it or opt out entirely.

Your first decision: engage or reject

Right away you have a choice:

  • Reject him outright if you do not want to deal with the multi-stage event. This is the safe, simple option.
  • Play through his choices for a potential reward. This takes more effort and attention but can pay off.

If you choose to engage, Barney will require coffee, treatment, and eventually a place to hide.

The key choice: police vs hide

The decision that defines the Barney event comes when he asks you for a place to hide from the police. You have two options:

  • Option 1 — turn him in to the police. Barney will knock out the police officer and run away. You avoid harboring a killer, but you get no reward.
  • Option 2 — hide him from the police. In return, Barney gives you a second coffee machine — a major upgrade that makes Sanity management much easier for the rest of your run.

For most players chasing a long, high-scoring run, the second coffee machine is well worth hiding him, since Sanity is the resource that limits how far you can go.

Barney can become an Anomaly

Be aware that Barney can turn into an Anomaly. If he does, you handle him exactly like any other threat — take him down with tasers, guns, or fists. So even while engaging his storyline, keep your defensive options ready in case the encounter turns hostile.

Should you do the Barney event?

It comes down to your goals:

  • Going for a long run or high score? Engage and hide him for the second coffee machine. The Sanity boost compounds over many shifts.
  • Struggling to stay alive, or in a chaotic moment? Rejecting him is perfectly valid — one less thing to juggle, and you avoid the risk of him turning Anomaly at a bad time.

Because Barney is multi-stage and asks for coffee, treatment and a hiding spot, he demands sustained attention across part of a shift. Do not start engaging him in the middle of an Ambulance rush or while a Death Ritual timer is running — handle the timed emergencies first, then give Barney the focus his storyline needs.
