In Animal Hospital, Sanity is your real health bar, and coffee and food are how you refill it. They are the most important consumables in the game because every threat ultimately attacks your Sanity, and running out ends your run. Think of coffee and food as your health potions and treat them accordingly.

How they work

You buy coffee and food from the shop with shift cash, then consume them to restore Sanity. There is no trick to it — when your Sanity dips, you drink or eat to bring it back up. The skill is in when and how much you keep on hand, not in the act of consuming.

Always keep a buffer

The golden rule of Sanity restoratives is to never spend down to your last coffee. This is because many Sanity losses are unavoidable:

The second coffee machine

The biggest single upgrade to your Sanity economy is the second coffee machine, earned through the Barney event after shift 2. If you choose to hide Barney from the police, he rewards you with it. Doubling your coffee access makes Sanity management dramatically easier for the rest of the run, which is why long-haul and high-score players almost always take the hide option. It effectively raises your Sanity ceiling for the cost of a single choice.

Pre-buy before risky moments

Smart players stock up on coffee and food before shifts they know will be demanding. The Ambulance event after shift 4 throws multiple Skinwalker threats and fires at you at once, all draining Sanity — so arriving with a full reserve is essential. Likewise, if you are about to engage the multi-stage Barney event or a known Sanity-heavy room, top up first.

First priority in the shop

In the overall shop priority, Sanity restoratives come first, ahead of even weapons. The logic is simple: you lose runs to zero Sanity more than to anything else, so insuring against that is the highest-value spend. A weapon helps you fight Skinwalkers, but coffee is what lets you survive everything else the hospital does to you.

Make checking your coffee supply part of your between-shift routine, the same way you would reload before a fight in any other game.
